The Contact Page allows customers to get in touch with your store through a contact form or direct contact information such as email, phone number, and address.
It is designed to help customers submit inquiries in a clear and convenient way.
The Contact Page is built from two main components, each serving a distinct purpose:
Page – Displays the static content of the page, created and managed directly in Shopify.
Contact form – Displays the contact form along with configuration options that allow customers to submit inquiries.
The Page is used to display content entered directly in Shopify under
Online Store → Pages.
This section is typically used to display the page title and a short introductory description above the contact form.
Create or edit a page in Shopify.
Enter the desired content.
Assign the page to the contact template.
Note: The Page section only displays content and does not contain the contact form.
The Contact form is the main section of the Contact Page.
It allows customers to submit inquiries directly to your store.
The Section header appears above the form fields and custom fields.
It is used to display a heading and description that introduce the contact form and guide customers before submitting their message.
By default, the Contact form includes three basic fields:
Name
Message
These fields cover the most common contact use cases.
You can extend the Contact form by adding Custom field blocks.
Each Custom field can be configured with:
A label
Field type: Text, Phone, or Options
Required state
Options (for option-based fields)
Multiple Custom field blocks can be added to collect additional information when needed.
The Contact form supports a two-column layout on desktop.
When the Desktop layout split option is enabled:
The left column displays content and store contact information.
The right column displays the contact form.
This layout helps balance the design and improves readability.
All messages are sent to the Sender email address configured in your Shopify store.

No. Shopify’s default contact form does not support file attachments.
If you need this feature, you must use a third-party app.

These items are configured using the Icon with text block and provide additional ways for customers to reach your store outside of the form.
Please check that:
The sender email address is correctly configured.
There are no typos in your notification settings.
